What should I bring to my first appointment?
Please bring your insurance card, a valid photo ID, a list of current medications, and any relevant medical records or recent test results. Arriving 15 minutes early will help us complete your intake paperwork smoothly.
What is your cancellation policy?
We kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Missed appointments without notice may be subject to a fee of $25.
